SUMMARY:Collaboration in Care Conference: A Virtual Experience
DESCRIPTION:The third annual Collaboration in Care Conference (CCC 2025) is a free\, virtual\, two-day\, national gathering designed to advance integrated\, cross-sector approaches to HIV and aging services. Participants will connect with peers and experts to share knowledge\, explore successful care models\, and co-develop strategies that enhance access\, quality\, and coordination of care for people aging with HIV. Attendees will leave equipped to identify key care components\, aging service resources\, and assess their organization’s readiness to implement integrated services.
